Your First Month in Spain: How to Use Your Health Insurance

You've arranged your policy, you've landed, and now a practical question appears: how do I actually use this? Many people hold insurance for weeks before they book a first appointment, and then discover they don't know where to start. This guide walks you through your first month so you're ready before you need care.

Step 1: Keep your documents in one place

Save your policy documents and your insurance card details in an easy-to-find spot, both digitally and on paper. You'll want your policy number, the insurer's contact details and the emergency assistance number to hand.

Step 2: Activate your card and download the app

Most insurers provide a physical or digital card and an app or online portal. Register as soon as you receive your login details. Through the app you can typically find providers, request authorisations, and book appointments.

Step 3: Read your policy summary

Spend ten minutes understanding the basics:

  • Whether you have copays and when they apply
  • Any waiting periods or exclusions
  • Which services need prior authorisation
  • What is covered outside Spain (See: How to Use Your Expat Health Insurance When Traveling Outside of Spain)

Step 4: Find providers near you

Use the provider directory (cuadro médico) in your app or on the insurer's website. Filter by:

  • Specialty, such as general medicine, paediatrics or gynaecology
  • Location, so the clinic is convenient
  • Language, if you prefer an English-speaking doctor (See: English-Speaking Private Health Insurance in Spain)

Because networks differ between plans, always confirm that a clinic is covered by your plan before booking. (See: Same Insurer, Smaller Network, Questionable Compliance)

Step 5: Book a first check-up

You don't have to wait until you're ill. A first appointment with a general practitioner helps you get familiar with the process, and means you already have a relationship with a doctor if something comes up later. Depending on your plan, you can book through the app, by phone or via the clinic.

Step 6: Know what to do in an emergency

  • Call 112 for emergencies.
  • Go to the nearest emergency department if it's urgent. Don't delay while checking whether it's in your network.
  • Contact your insurer's emergency line as soon as you can afterwards.

Write down the nearest hospital to your home and the insurer's helpline number now, so you don't have to search under stress.

Step 7: Understand authorisations and claims

For certain tests, procedures or hospital stays, your insurer may need to authorise the service first. This is usually quick when requested through the app or your provider. If you ever need to make a claim, keep invoices and reports and follow your insurer's process.

Step 8: Add the other protections that matter

Health insurance protects your health, but life abroad involves other risks too, such as your home, belongings and any pets. (See: Renewing Your Coverage: What Residents Should Know, and Why Health Isn't the Only Protection You Need)

First month checklist

  • Note the nearest emergency hospital
  • Put your renewal date in your calendar
