Medical insurance should fulfill the following requirements:
Operates in Spain

Some of the requirements above are easy to understand, but some of them seem confusing. We will explain some of the points in depth in the following.
1. Operating in Spain
This is an understandable requirement as it’s assumed that you will live in Spain. You may recieve an offer for EU medical insurance, don´t assume that it will cover Spain! Instead, you should check throughly to see if this insurance covers Spain or not. Often, Spain and a few other countries in Europe are excluded from some travel insurance providers. This is because that Spanish healthcare is more expensive than other EU countries like France or Germany. Some insurers want to cut down costs to make it seem like you are getting a better deal but in the end the insurance won´t cover what you need.
This is a term that is not familiar to most expats. A copayment or deductible is a type of fee you must pay every time you go visit a doctor or use any health services. If you want to have qualified visa insurance, it should grant you free of charge when you use medical services such as seeing the doctor, using the ambulance, and taking a medical test.
The rationale of a co-payment plan is that it encourages people to use medical and health services only when it’s necessary, leaving precious medical resources to the people that need treatment. However, with a non-copayment plan, you can enjoy the medical services without worrying about any additional fees. This is a different rationale to encourage people to care for their health more often by using reliable means.

3. Repatriation of remains
Another requirement is that the medical insurance should ensure to transport the mortal remains of the insured person back to the loved ones living in the country of origin. This is a small-possibility incidence, but qualified visa insurance should be able to offer this service for free when anything unexpected happens.
Many insurers offer all the other services but cannot fulfill this requirement because they cannot afford to do so. If you contract insurance that cannot fulfill the requirement, you will have a high possibility of being rejected by the consulate after submitting your visa application.
